In-person Sierra Vista Registration

In-person registration, if you choose not to register online, will be held January 7th, 21st, and 28th from 10:00 to 2:00 at the Mall in Sierra Vista. Cash or card accepted.

Please be sure to bring the required forms necessary to complete registration. The SCHOOL ENROLLMENT FORM IS THE ONLY FORM YOU NEED and it will replace the child's birth certificate, and the three proofs of residency required for Minors and above. (T-ball and Coach Pitch need NO forms but ONLY show proof of age).

The required forms can be a government issued ID card, utility bill, insurance card, etc...1 needed from each of the 3 groups in the link below...
